Medical (military medical) commission

The main task of the medical (military-medical) commission of conducting a military medical examination in order to determine the state of health, physical development, and for certain types of service activities - according to the individual psychophysiological characteristics of the suitability of candidates for service in the National Police, candidates for admission to higher educational institutions of the Ministry of Internal Affairs, the suitability of police officers for further service when moving through the service and dismissed from service. Determination of the suitability of candidates for military service under the contract, in the military reserve, candidates for entry into higher military educational institutions of NSU, and the suitability of servicemen upon dismissal.

IHC in accordance with the tasks assigned to it:

1) conducts a medical examination of candidates for police service, police officers and former police officers, candidates for admission to higher educational institutions, and applicants for the higher education of educational institutions;

2) conducts a military medical examination of candidates for military service under a contract, in the military reserve, before entering the university, servicemen of the NSU, former military personnel, former privates, and commanding officers;

3) provides control over the conduct of medicinal and military medical examination, as well as medical and diagnostic work in health care institutions of the Ministry of Internal Affairs and NSU;

4) summarizes and analyzes the results of the drug and military medical expertise and develops proposals for improving this work;

5) together with medical specialists of health care institutions of the Ministry of Internal Affairs, summarize the results of treatment and medical examination and assess the impact of the conditions of service on health status and suitability for service.

6) determine the causal relationship of diseases, wounds, injuries, contusions, and injuries that resulted in the death of police officers, military personnel, former police officers, former military personnel, former privates, and commanding officers.

Military medical examinations regarding candidates for the police service, candidates for training in higher educational institutions of the Ministry of Internal Affairs, police officers, former privates, and commanding officers are carried out in accordance with the order of the Ministry of Internal Affairs of Ukraine on 03.04.2017 No. 285. Regarding candidates for military service under the contract, in the military reserve, candidates for admission to the higher military educational institutions of NSU, military personnel, according to the order of the Ministry of Defense of Ukraine on August 14, 2008 No. 402.

 

List of medical documents for persons entering the service of the National Police of Ukraine

Referral from the personnel department to a medical examination in the form (Appendix 4).

Military ID or certificate of registration for men, passport for women.

Outpatient card from the place of residence.

A detailed extract from the medical card of the medical institution at the place of residence, work, service, or study for the last 5 years. (The extract must be certified by the signature of the doctor and the seal of the medical institution).

Certificate or certificate of a psychiatrist and narcologist (at the place of registration).

Certificate of mandatory prophylactic vaccinations (2 pcs. Form 063 / y).

Common analysis of blood, urine, an. blood glucose, an. blood test, hepatitis B, C; HIV.

X-ray (fluorographic) examination of the chest organs.

Note: Former military personnel must have a medical certificate, a copy of the M (MM)C Creport, and a copy of an extract from the dismissal order. Police candidates must submit documented results of the specified examinations carried out within one month prior to the medical examination at M (MM) C.

 

The list of medical documents for persons entering the higher educational institutions of the National Police of Ukraine

Referral from the personnel department to a medical examination in the form (Appendix 4).

Certificate of registration or military ID for men, passport for women, 1 photo 3x4.

Outpatient card. Lyceum students must submit a medical profile.

A detailed extract from the medical record of the medical and preventive institution at the place of residence, work, service, or study for the last 5 years. (The extract must be certified by the signature of the doctor and the seal of the medical institution).

Certificate (certificate) of a psychiatrist and a narcologist (from the place of registration).

Certificate of mandatory preventive vaccinations (in form 063 / y) in 2 copies.

General analysis of blood, urine, blood glucose, and blood test for RV, hepatitis B, C, and HIV.

X-ray (fluorographic) examination of the chest organs.

Note: Applicants for training must provide documented results of the specified examinations carried out within one month prior to the medical examination at M (MM) C.

 

List of medical documents for persons entering military service under a contract in military units of the National Guard of Ukraine

  1. Military unit Commander`s guidance for a medical examination according to the form (Appendix 14).
  2. Military ID or certificate of registration for men, passport for women.
  3. Outpatient card from the place of residence.
  4. A detailed extract from the medical card of the medical and preventive institution at the place of residence, work, service, or study for the last 5 years. (The extract must be certified by the signature of the doctor and the seal of the medical institution).
  5. References (certificates) of a psychiatrist and a narcologist (at the place of registration).
  6. Certificate of mandatory preventive vaccinations (in form 063 / y) in 2 copies.
  7. Certificate from the anti-tuberculosis dispensary.
  8. General analysis of blood, urine, blood glucose, blood test for RV, hepatitis B, C, HIV, blood group, Rh factor, biochemical blood test (gen. Bilir., ALT, AST).
  9. X-ray (fluorographic) examination of the chest organs.

Note: Former military personnel must have a medical certificate, a copy of the M (MM) C report, and a copy of an extract from the order of dismissal from service. Candidates for military service under contract must provide documented results of the specified examinations carried out within one month prior to the medical examination at M (MM) C.
